Seeking Respite Providers- Collin County

LifePath Systems is seeking providers to contract with the Center to provide respite care services for individuals with Mental Retardation, Autism or PDD.
Potential providers must be 18 years of age, must pass a criminal background check, nurse aid and misconduct registry check, and must complete a contract and required training prior to service delivery. Reimbursement rates range from $10-$18 per hour depending on client need. Potential contractors must provide a driver’s license or other proof of identity, a Social Security card and auto insurance if they are providing transportation.
